Home Service Coordination Community Habilitation Respite Family Reimbursement Outreach Services

This home-based service provides training on daily living, communication and socialization skills for eligible persons. 
Planned goals are individually set based on needs and goals expressed by the consumer or care-giver.  Available to eligible Medicaid recipients.
