QPRC provides Medicaid Service Coordination to qualified individuals.  The consumer chooses a service coordinator to work with them and their family to assess and plan for individual needs.  The service coordinator works to assure optimal independence and integration into the community while ensuring that the person is availing themselves of eligible entitlements and services.
